Supplemental Rules - The MALA Rules and Regulations Committee have developed supplemental information to help clarify rules and to set guidelines and/or regulations for teams and clubs.
 

Medical Forms

Consent to Treat Form - This Form 1 of 3 to be completed for registration. This form needs to be completed before official practices are scheduled to begin on March 1, 2010. A new form needs to be completed before every season.
MALA Registration Form - This Form 2 of 3 to be completed for registration. This form needs to be completed before official practices are scheduled to begin on March 1, 2010. A new form needs to be completed before every season.
Player Medical History Form - This Form 3 of 3 to be completed for registration. This form needs to be completed before official practices are scheduled to begin on March 1, 2010. A new form needs to be completed before every season.
